RELEASE OF YOUR NAME AND ADDRESS

DONOR RELEASE INFORMATION

The Office of Personnel Management (OPM) requires that the donor must specifically indicate his or her desires with regard to the release of their name and address to the agencies to which they designated.

Marking one of the following options will result in:

• DO NOT RELEASE ANY INFORMATION
  This will result in no release of your name & address to any charities.
  Your information is NOT RELEASED.
  
  
• RELEASE MY NAME ONLY TO THE CHARITIES I DESIGNATED
  This will result in your name being released to the charities you designated.
  Your information IS RELEASED.
  
  
• RELEASE MY NAME AND CONTACT INFORMATION TO THE CHARITIES I DESIGNATED
  This will result in your name, home address, and e-mail address being released to all the agencies you designated.
  Your information IS RELEASED.

If both e-mail and home address options are marked and both an e-mail and a home address are provided your e-mail address will be released to the agencies you designated.

NON-RELEASE OF NAMES AND ADDRESS

Should any of the following occur, your name and address WILL NOT be released:

* If no option is marked
* If all options are marked
* If no e-mail or home address is provided
