The Office of CFC Operations is pleased to
release the pdf fill-in versions of the 2010 CFC
Local
Independent, Federation, and FSYA Applications. These
are now available for downloading. You will need to have Adobe Acrobat
Reader installed on your computer to view these files. If you don't have
Reader yet, get a copy
from Adobe. For more information, please review Memorandum 2006-22.
2010 CFC Applications being accepted from March 1, 2010 to April 1, 2010
All you need to do is:
- Complete the application in its entirety.
- Attach any documentation requested.
- If you are a DBA please submit documentation as requested in the instruction.
- Sign the application – Original documents and signatures are required. Faxes
and electronic submissions cannot be accepted.
- Applications that ARE NOT COMPLETED will not be considered.
