This page describes how to fill out the Pledge Form.

PROCEDURES:

  1. Obtain a PLEDGE FORM and a 2010 Central California CFC Donor Guide from your Activity Coordinator or Key Worker. Make all contributions using only a 2010 Central California CFC Pledge Form and 2010 Central California CFC Brochure. Do not use another year's CFC pledge form or brochure.

  2. ALL DONORS: Complete sections 1-5 of the pledge form.

  3. PAYROLL DEDUCTION DONORS: Complete section 6 of the pledge form.  Payroll Deduction contributions require you to provide your Social Security Number (SSN) and to sign the pledge form. Cash or check contributions do not require your SSN or a signed pledge form.

  4. ALL DONORS: Turn in the white and yellow copies of the pledge form to your Coordinator or Key Worker. (Include cash or personal check if appropriate)

  5. ALL DONORS: Retain the "Contributor's Copy" of the pledge form for your personal records. (pink)

  6. CONFIDENTIAL GIFT: Any donor can make his or her contribution a confidential gift. The donor needs only to place all copies of their pledge form (less the pink copy) into an envelope, mark the envelope "confidential gift" and turn it in to your Coordinator or Key Worker. The envelope will not be opened until it arrives at the CFC Audit Point where an audit clerk will open the envelope and record the pledge for your activity's credit. Note: Should you elect to use payroll deduction as a confidential gift, a degree of confidentiality will be lost since the yellow copy will be forwarded to your Coordinator for delivery to your payroll office.

  7. DESIGNATING MORE THAN 5 AGENCIES: Should you desire to contribute to more than 5 agencies, complete sections 1 and 3 only of a second pledge form and staple the white copies together. Retain the pink copy of the second form for your records.   Turn in the white and yellow forms to your Key Worker or Coordinator. 

  8. IRS AUDIT REQUIREMENTS: In case of an IRS tax audit, your pink copy of the pledge form will be required for verification of a payroll deduction contribution. A cancelled check will suffice for a donation made by personal check. A letter from the CFC office confirming a cash donation will be required for a cash contribution. Note: The CFC office retains pledge forms for three campaign years.
